University of Arkansas

Under Academic Policy 1900.10, the Survey Research Center serves as the sole provider on campus of disaggregated data from the University of Arkansas for use in scholarly research. This service is intended primarily for the UA community and is in place to protect the privacy and confidentiality of the same. There is a service fee based on the complexity of the sample drawn.

To request that the SRC extract data from the Data Warehouse or other University sources, the requestor should:

1. Apply to the UA Institutional Review Board for a review of the treatment of human subjects in his or her studies using this form
2. Read and sign the data confidentiality form
3. Complete a Data Warehouse application form, including the following documentation:
   • The proposal of study
   • Study time line
   • A copy of the original IRB application
   • A copy of the original IRB approval
   • Copies of any IRB amendments
   • Approvals of any IRB amendments
   • A letter from the committee chair or faculty advisor, if applicable.
4. Complete a billing form
